The contestant must slide down two cables while standing on a small platform. One at a time, contestants pick gifts from under a Christmas tree. Contestants must swim underneath a 20 foot by 40-foot Plexiglas grid that is lit on fire.

Contestants hold onto monkey bars, suspended under a helicopter. In a Plexiglas coffin and covered with leeches. Contestants search for rubber boots at the bottom of a tank of cut up fish parts. Contestants must run their motorcycles off of a cliff.

Contestants will start on the roof of The Mandalay Bay. A tank is filled with Piranha and pig kidneys, which will need to be picked up off using their mouth. The contestants are allowed to eat up to six cave spiders.

Taxi cabs will each be suspended from their own crane. Player is placed inside a sealed Plexiglas coffin with thousands of roaches. The contestants will be hung by their ankles from a crane and suspended over a swimming pool.

Contestant seated in the helicopter will "go" they crawl out and under the helicopter. Players transfer cow hooves out of a tank of Cod Liver Oil and into a tank of Maggots. Contestant must drive their car at approx. 25mph towards another car.

Contestants will start this game floating in the water. Contestants compress a cows eye with their mouth, squeezing the juice out of the eye into a glass. Contestant will stand on Euclid dump trucks as they travel down the road.

Players are placed into a large plexiglas coffin with several thousand snakes. Eight glass torch-goblets are placed on stone pillars of varying heights. Contestants will bob in a tank filled with cow eyes to recover pig hearts.

Helicopter picks up contestant by their ankles then dunks them. Contestants transfer a bunch of Fear Factor items off of a moving conveyer belt and onto a scale. Contestants will stand inside a giant ring above water.

Contestant must dive into the water and swim down to a city bus. On a table, there are dishes of roaches. Players are given fifty cow brains with a FF chip hidden inside. Contestants must make the way across a maze of beams.

The contestant is placed in a specially designed clear bag. A tank is filled with millipedes, roaches and worms. Contestant is standing on one of four 8-foot tall 16-foot long Plexiglas walls suspended high above the ground.

Contestant will be riding in the back of a truck driving down a pier. Players will have to clear tables to uncover a three-digit combination written on the glass top of each of the tables. Contestant must drive their car under a semi truck.

There will be a boat that is towing three rafts. A Plexiglas bottom covered in pig parts. The child stands under a large metal shower-head and pulls a chain. Suspended above the water. The kid must climb through the tubes and traverse.

Contestants must leap back and forth from the boats to get as many flags. Contestant is put into a vat used for stomping on grapes to make wine. Contestant must walk the length of the truck removing flags along the way.

Contestants will be placed on a platform floating in the water, where they will grab a skid of the helicopter. Player is chained between two poles and covered with bees. Contestants will be seated in the front of a car suspended over the water.

Contestant will be contected to a dune buggy by his ankles. Contestant will have to bury their spouse's head in worms using only their mouth. One person from each team will be hanging from the skids of a helicopter.

Contestant will be chained to either side of a platform with the contestant having one key to free them from chains. Player dunks their head into a tank and retrieves a cows heart with their mouth.

Fear Factor - Spouse Spin, Fear Factor Pie

In this couples stunt, partners will hang by their ankles, they will each hold onto one end of a rope and be lifted over the water. Couples will use their mouths to root around in pies made from wax worms, pigs stomach, fish sauce and duck tongue.

Player A is placed on a dunk tank. In the tank is a mixture of cow parts floating in water. One member of the team will be suspended by their ankles underneath a helicopter.

In this stunt, the female contestant will be placed in a cage that is hung from a helicopter and is approx. 20 feet in the air. A dumpster filled with disgusting garbage is placed at one end of an alley.

Contestant will be standing on a beam that is suspended high in the air. Player is placed in a coffin with 100 chicken feet and is then covered in rats. Couples stunt one of the team members is hog tied by four ropes and suspended.

Two contestants are seated in the front of a car. The driver is blind folded and the passenger must navigate for them. Player is strapped down in a coffin and then covered with 500 Tarantulas.

Contestant will exit a helicopter and release a flag. They will be standing on platforms suspended in the air. Contestant must chew through the cow intestine. Contestants will ride a mini motorcycle across a beam that is suspended from a crane.

Contestant must get on a pole directly out, under the nose of a helicopter. Players will bob in cheese and then eat grasshoppers. Contestants are hung upside down into a Plexiglas tank filled with Eels. Contestants must drive up with only two wheels.

The contestant will be put in a clear plex-glass tube that will be lifted up and placed just above the water. Snakes and lizards will be dumped in the box. Contestant will be standing on a plexi-glass incline pirtched on top of a roof in the air.

Contestant will be standing in either end of two large cylinders, suspended in the air over water. Player must grab a mouth full of insects, then place into a meat grinder using their mouth. Inside the tanker truck, it would be filled with water.

Contestants are placed in a box with two plexi-glass walls that will be lifted off the ground. Contestants will be served live snails. Contestants are seated in a car that is in the back of a semi truck.

Contestants will be dangling from a rope off the roof. Contestants will have to clear a bug splattered car windshield using only their mouth and face. The contestant will be standing on a beam suspended from a crane.

Contestant will stand on top of a truck as it speeds trying to grab as many flags. Contestants will shoot potatoes at a target using a shoulder mounted air cannon. The contestant must start the car, there are tractors driving towards their car.

Contestants will be standing on a platform in the air. Contestants will have to play miniature golf to determine the size of a sea cucumber they will have to eat. Contestants will be seated on the back of a jet ski driven by a professional driver.

Contestant will be wrapped in a vest of chains. The contestants will crawl, climb and swim through an obstacle. In this stunt the contestant will be sitting in a helicopter flying approx. 20' over the water.

Two contestants will be standing on a platform on top of a building. Players sit in two boxes, one stacked on top of the other. For this final stunt three co-ed couples will be placed on a tight rope and suspended over the water.

The contestants will be standing on a beam over the water. Contestants will have to get mouthfuls of blended worms and transfer them into a container with house flies. Contestants must jump their car over al flat bed train cars.

Contestants will be standing in a cylinder that is suspended just above the water. Contestant is handcuffed and placed in a clear body bag filled with various types of bugs.

The parent and the child will have to cross a beam above the water releasing flags along the way. Cockroaches are being poured into the box while the child sits in a box without a top. In this family stunt the child will start by standing on the dock.
